Abstract

Debatable data on prevalence of coronary artery disease in liver cirrhosis are presented in the review. Influence of etiology of liver cirrhosis, of other risk factors on possibility of development of coronary atherosclerosis in terminal stages of liver pathology is considered. Necessity of conducting researches for purpose of specification of frequency of development of coronary artery disease in liver cirrhosis, of its interrelation with features of hepatic pathology, of possibilities of improvement of risk stratification and working out of treatment strategy of patients, including those expecting liver transplantation is noted.

Abstract

Among structural heart abnormalities (SНA) in clinical practice, mitral valve prolapse, an open oval window, and atrial septal aneurysm, which can present a real threat to the health and life of patients, are most common. Frequent subclinical course makes it difficult to detect them in a timely manner and requires focused attention of practicing doctors in assessing the phenotypic features of the structure of the musculoskeletal system and connective tissue, which can serve as markers of latent defects in the development of the heart. The article provides data on the most typical symptoms and clinical manifestations of SНA, possible complications and therapeutic tactics. Early detection of structural abnormalities of the heart and the choice of the right tactics for passive or active monitoring of patients with varying degrees of severity of defects in the development of the heart will help preserve the health or even life of patients.

Abstract

The study showed that the eccentric myocardial hypertrophy predominates in the group of patients receiving hemodialysis, which is mainly due to volume overload of the left ventricle, while in the kidney transplant recipients group, in the vast majority of cases, concentric hypertrophy of the left ventricle is detected due to the predominance of pressure load on the heart. Therefore, the type of remodeling of the left ventricle in patients with terminal renal failure can be determined by the type of renal replacement therapy.

Abstract

The high prevalence of Helicobacter pylori infection and the low efficiency of its eradication in real clinical practice motivate to search for ways to optimize the used treatment regimens. The article highlights data on prevalence, transmission routes, modern methods of diagnosis and treatment of Helicobacter pylori associated diseases. The experience of real clinical practice based on the data of the territorial register of eradication therapy in Yekaterinburg in 2018-2019 is given. When analyzing the ambulatory charts of 437 patients, the efficacy and safety of standard triple therapy with bismuth was studied compared to the combination of standard triple therapy with bismuth and a metabiotic based on Lactobacillus reuteri DSMZ17648. Additional administration of metabiotics increased the eradication efficiency by 5.9%, and the frequency of adverse events decreased by 18.1%.

Abstract

242 patients with increase of body mass index were examined, of them, in 180 patients, fatty hepatosis was revealed. In the course of the examination, we defined manifestations of metabolic disorders in the patients: obesity, disorders in glucose tolerance, dislipidemy, arterial hypertension. In the course of treatment, in the experimental group, significant effects were achieved after application of ursodeoxycholic acid — hypolipidemic, weight loss, mostly pronounced in the application as a modifier of food behavior, psyllium preparation.

Abstract

Drug lesions of the liver account for about 10% of all side reactions of the macrobody associated with the use of pharmacological preparations. The article describes the main mechanisms of drug injury to the liver. Diagnosis of drug liver damage is often difficult and requires the physician not only to carefully collect the history of the patient with the developed disease, but also to have a good orientation in its clinical and morphological manifestations. A clinical case of drug cirrhosis of the liver is given, when the patient for many years overused various non-steroidal anti-inflammatory drugs, which led to a fatal outcome.

Abstract

The data concerning physiological need, recommended daily consumption of vitamin D, provision of children with this vitamin were analyzed, as well as the methods of correction of its deficit by using food products enriched with cholecalciferol and food supplements. The data on enriched food products used in children’s nutrition and content of vitamin D in them were presented. Criteria of relation of vitamin D to food supplements and medications were discussed, basing on composition and dosage of cholecalciferol. Abstract

The questionnaire survey studied the prevalence of osteoporosis and low-traumatic fractures and the level of knowledge about osteoporosis in 600 men and women aged 50 years and older undergoing inpatient rehabilitation treatment. Low level of knowledge about osteoporosis and insufficient frequency of prescription of antiosteoporotic therapy among patients with the previously defined diagnosis were revealed, which indicates the need for widespread use of educational programs as part of comprehensive medical rehabilitation to prevent new osteoporotic fractures.

Abstract

The results of clinical monitoring of the effectiveness and safety of therapy in adult patients with atopic dermatitis using various dosage forms of methylprednisolone aceponate are presented. The rationality of differentiated prescription of dosage forms of methylprednisolone aceponate in the localization of skin manifestation in the problem areas, on the skin of the torso and limbs, and according to the clinical manifestations is shown. The safety of the drug methylprednisolone aceponate in the treatment of patients with atopic dermatitis, the absence of side effects in the treatment at the specified time was confirmed.
