The purpose of the review was to study the data of the modern literature on the effect of diet on the skin condition and the course of some dermatological diseases. Recent studies have shown a strong link between diet and a number of dermatological conditions. Food allergies and Western eating patterns can dramatically alter microbiome composition and intestinal permeability. In conditions of disturbed microbial balance and changes in the barrier function of the intestine, the penetration of immunogenic molecules into the systemic circulation, including food antigens, bacterial toxins and pathogens, which can accumulate in the skin, disrupt the epidermal barrier and contribute to chronic inflammation, increase. However, in children with atopic dermatitis, elimination diets did not show a significant advantage in reducing the severity of the disease; on the contrary, adverse events associated with malnutrition, lack of vitamins and minerals were more often recorded. Despite the fact that dietary intervention is an important aspect of the treatment and prevention of a wide range of dermatological diseases, many questions remain open today.

The incidence of dermatoses of the peoples of the North is closely related to the way of life, traditional trades (reindeer husbandry, fishing, gathering wild plants), the impact of adverse climatic factors, as well as the influence of industrial enterprises on the fragile nature of the North. Along with the problem of anthropogenic and technogenic pollution of the environment in most regions of the North, there is also the problem of the widespread occurrence of various biogeochemical anomalies, among which the leading place belongs to natural iodine deficiency and the widespread occurrence of iodine deficiency diseases. A territorial feature that is seasonal in nature is allergic dermatitis caused by insect bites (mosquitoes, midges), as well as skin rashes associated with climatic and geographical factors (cold urticaria, simple contact irritable and photocontact dermatitis, etc.). The article gives a review of the dermatologic morbidity structure for one of small peoples of the North — the Selkups. Urgency of the problem of healthcare for indigenous and small peoples of the North was indicated, main climatic and geographical factors of the Selkup habitat were shown, which directly influence on the skin condition. In retrospect, according to medical documentation, the data of annual reports of the skin and venereologic dispensary in the city of Salekhard, the central district hospital of the Krasnoselkupsky district, the results of medical examinations and outpatient visits to a dermatovenerologist in the city of Noyabrsk and the village of Krasnoselkupsk for 3 years were analyzed. The object of the examination was patients — Selkups, divided into nomadic and sedentary groups.
The article estimates the frequency of detection of concomitant somatic pathology in patients with psoriasis. The study included 107 psoriasis patients aged 19 to 75 years and older. Anamnesis of the disease was analyzed, which contains information about the causes of the disease and its duration, previous treatment, and trigger factors. The clinical method of the study included a General clinical examination of the patient, including consultations with related specialists to conclude the presence of concomitant somatic pathology or clarify the severity of an existing disease and recommendations for treatment and dynamic follow-up. The study was carried out clinical evaluation of the skin and mucous membranes, assessment of localization skin process, nature of the disease, definition of limitations, seasonality, duration of psoriasis, the effect of earlier therapy, time of recurrence. According to the results of the study, 83.1% of the examined patients have somatic pathology, which is dominated by pathology of the cardiovascular system, digestive organs, respiratory system and ENT organs. The features of the distribution of comorbidities in patients with psoriasis, which should be taken into account when drawing up a treatment plan for such patients, were revealed.
Psoriasis is a common chronic dermatosis associated with metabolic syndrome and its components. Metabolic syndrome contributes to severe psoriasis. Obesity is a multifactorial disease and its development is influenced by genetic factors. Carriers of the FTO gene risk allele are associated with more severe psoriasis, higher risk of obesity, psoriatic arthritis and metabolic disorders. Nutritional intervention plays an important role in psoriasis and co-morbid metabolic syndrome management. The article reviews current data regarding dietary habits of psoriasis patients, impact of weight reduction and dietary supplements administration on psoriasis.
Systemic lupus erythematosus (SLE) is an autoimmune disease, the etiology of which remains unknown. Skin syndrome in SLE is often of paramount diagnostic value – skin lesion is the earliest symptom in 20-30% of cases, and in 60-70% it manifests itself at various stages of the course of the disease, which leads to the patient's primary referral to a dermatologist. Frequent diagnostic errors are due to the absence of clinical symptoms of systemic lesions in the presence of cutaneous manifestations of lupus erythematosus (LE). In this case, laboratory tests are an integral part of the algorithm for managing patients with LE at the stage of contacting a dermatologist. The most specific laboratory test is the determination of antinuclear antibodies by an indirect immunofluorescence reaction, referred to as antinuclear factor (ANF). An elevated ANF titer is included in the list of diagnostic criteria for SLE, its detection allows one to suggest a diagnosis of SLE and determine a further algorithm for the provision of medical care. The article presents two clinical cases illustrating that against the background of an isolated skin lesion and the absence of visible somatic pathology, abnormalities in immunological tests were identified. This made it possible to suspect the systemic course of the process at an early stage and refer patients to a rheumatologist's consultation, where the diagnosis of SLE was confirmed.
Chronic spontaneous urticaria (CSU) is characterized by wheals and/or angioedema arising spontaneously and persisting for at least 6 weeks. The recommended treatment options include second generation H1-antihistamine drugs (sgAH, as first- and second-line therapy), omalizumab and cyclosporine. Revealing of biomarkers of the response to treatment and their implementation into clinical practice may facilitate the treatment effectiveness and decrease the frequency of adverse events. The review of literature was conducted in MEDLINE/Pubmed electronic databases using the keywords «predictors AND treatment AND chronic urticaria», «biomarkers AND treatment AND chronic urticaria». 25 articles were included. It was found that baseline eosinopenia, basopenia and/or high level of C-reactive protein might predict the poor response to sgAH. Baseline eosinopenia, low levels of serum total IgE, positive results of autologous serum skin test, histamine release test and/or basophil activation test are associated with the poor response to omalizumab but good response to cyclosporine. Potential biomarkers of response to treatment in patients with CSU have been reported in the literature. However, randomized clinical trials are required before implementing the biomarkers into the routine clinical practice.
This article shows current information about etiology, epidemiology and clinic of рarvovirus B19. Parvovirus infection is an acute infectious disease that is associated with a range of clinical conditions. The causative agent of the disease is parvovirus B19, which is tropic to the progenitor cells of the red bone marrow. The most common mechanism of transmission is aerogenic, but there are also vertical and hemocontact mechanisms. Infection occurs by airborne, transplacental pathways, as well as by hemotransfusion. High-risk groups include people with congenital hematological diseases, immunosuppressive individuals, and pregnant women. Parvovirus infection during pregnancy is particularly dangerous. The virus causes blockage of fetal erythropoiesis, which leads to anemia, tissue hypoxia, noninflammatory dropsy, and fetal death. The most common symptoms of parvovirus infection are the development of infectious erythema, arthropathies, and transient aplastic anemia. Diagnostics of parvovirus infection includes isolation of the pathogen's DNA in the blood in the PCR reaction, as well as the use of the ELISA method with the detection of IgM and IgG in the acute period of the disease. Currently, etiotropic therapy and specific prevention of parvovirus infection are not available, and patients receive symptomatic therapy. The prognosis in most cases, is favorable. This article provides an overview of a clinical case of parvovirus infection in a patient admitted to the Infectious Diseases Clinical Hospital Two with complaints of fever and exanthema.
The article discloses a modern definition of the immune response. The phylogenesis of immunity constitutes a single system and is an integral part of the development of multicellular microorganisms. The emergence of multicellular organisms contributed to the formation of a separate community of cells that would be responsible for maintaining homeostasis of the internal environment of the body. The interaction of these cells was a prototype of modern immunity, and the immune response became a form of regulation of the constancy of the internal environment. The role of the immune response in the body is as follows: the search and elimination of foreign particles, both exogenously penetrating (pathogens of infectious diseases) and endogenously formed (cells infected with viruses, tumor cells). The role of the innate and acquired immune response is determined. Particular emphasis is placed on the pathogen recognizing receptors, on their various types. The concept of a pathogen of recognizing receptors is disclosed, their interaction and activation is shown for various types of pathogens. Modern perceptions of interleukins and transcription factors are characterized.
In according to the most common definition, pseudotuberculosis is an acute zoonotic infection characterized by a polymorphism of clinical manifestations, prone to recurrent and protracted course. It is known that children suffer from pseudotuberculosis 4-5 times more often than adults. In recent years, there has been a trend towards an increase in recurrent forms of the disease. The lack of a unified clinical classification and clinical guidelines for the management of pediatric patients with pseudotuberculosis infection makes difficult to apply adequate approaches to the diagnosis, treatment and rehabilitation of this infection. The study of the clinical manifestations of pseudotuberculosis in the age aspect, duration and possible outcomes of the disease is a prerequisite for solving this problem. In this regard, prospective monitoring of 60 pediatric patients with a verified diagnosis of «Pseudotuberculosis» who received treatment for this infection in Novosibirsk children’s city Hospital number 3 in 2018-2019 was carried out. The analysis underwent: basic epidemiological data, the effectiveness of the etiological verification of the diagnosis by various methods. Clinical and laboratory features of the course of pseudotuberculosis were identified in the age aspect (by comparing the obtained indicators in two clinical groups: children under 3 years old and children older than 3 years). To assess the condition during the convalescence period, a face-to-face survey of 27 patients and/ or their legal representatives was carried out (in accordance with age characteristics). Patients with a history of pseudotuberculosis with a history of 6 months were selected for questioning.
The study discusses issues of the validity and efficacy of the proposed scheme of conservative treatment of clinically significant stages of osteoarthritis of the knee joint (II-III degree classification Kellgren – Lawrence), including a combined administration of drugs from different pharmacological groups: non-steroidal anti-inflammatory drug aceclofenac, a derivative of hyaluronic acid and cartilage chondroitin sulfate. The aim of the study was to assess the effects of the combined use of aceclofenac, hyaluronic acid derivative and chondroitin sulfate on the functional status of the affected joint throughout the study (30 days), as well as the progression of the degenerative inflammatory process by assessing objective data (ultrasound pictures and laboratory parameters). The prospective open-label study of combined conservative therapy in 69 outpatient patients for the period from 2017 to 2019 with a subject assessment of the articular component of the treatment was conducted. As a result of the study of the clinical efficacy of the above treatment regimen in patients with a predominance of the arthritis component of the disease, positive results of drug therapy were established. By the end of the observation the intensity of a pain syndrome and, in most cases, laboratory and morphological changes showed statistically significant positive changes, while positive dynamics of movements, observed mainly in patients with a second stage of gonarthrosis were not always reliable. None of the patients was excluded from the study, all patients underwent a course of therapy satisfactorily, no adverse events occurred.
The aim was to evaluate the effect of the combined butyrate and inulin supplement administered as an adjuvant to Helicobacter pylori eradication therapy (ET) on treatment outcome. 349 patients with chronic gastroduodenal diseases and infected H. pylori were еxamined. A comparative analysis of the effect of a combined butyrate and inulin supplement on improving the effectiveness of the optimized ET regimen, improving its tolerability and patients’ compliance. The success of eradication was controlled by a 13C urease breath test. Significantly higher eradication rates were observed in the ET + combined butyrate and inulin supplement group (91,3% versus 74,4%, р˂ 0,05), compared with the control group (ET only). A lower incidence of side effects (bloating, rumbling, diarrhea, bitterness in the mouth) by the end of the 14-day course of treatment was noted in the ET + combined butyrate and inulin supplement group (p˂ 0,01). Significantly higher patient adherence rates (87% versus 63%, p = 0.04) and a lower discontinuation rates of ET (χ2 = 8.22, p = 0.0041) were also noted in the ET + combined butyrate and inulin supplement group, compared with control group (ET only). The adjuvant administration of a combined supplement of butyric acid and inulin to a standard three-component ET regimen significantly increases the effectiveness of treatment and is associated with less self-reported side effects (diarrhea, bloating, bitterness in the mouth), lower side effects-related treatment discontinuation rates and higher treatment compliance.
The Russian National Congress of Cardiology 2020 was held from September 29 to October 1. Due to the unfavorable epidemiological issues congress switched into online – 149 meetings in 15 virtual rooms. All the meetings were recorded and uploaded to the YouTube channel of the Russian Cardiological Society. There was a decent discussion about cardiology and multidisciplinary interaction. Special attention was payed to recently updated ESC Guidelines on Atrial Fibrillation 2020. Over the past 10 years, anticoagulant therapy has gone through a series of major events. The new guidelines have been generally well approved by both medical community and patients. Comprehensive assessment of risk factors in patients with atrial fibrillation is a key for the prescription of adequate treatment considering effectiveness and safety. The new era of direct oral anticoagulants was represented at satellite symposia by opinion leaders in cardiology.
