Transient ischemic attacks (TIA) are a strong predictor not only of acute cerebral circulatory disorders, but also of other vascular events, including acute coronary syndrome and arterial thrombosis of other localization. Current clinical guidelines do not distinguish between TIA and small ischemic stroke in terms of treatment strategy. Meanwhile, the high probability of intracranial hemorrhages in patients with small strokes and small artery disease limits the possibility of aggressive therapy in patients with TIA. This contradiction can be resolved if TIA is assessed as an emergency condition similar to acute coronary syndrome. Modern methods of brain imaging solve the problem of differential diagnosis between TIA and small strokes. Aggressive therapy with antiplatelet agents, antihypertensive agents, and statins reduces the likelihood of fatal vascular events in patients with TIA.
In order to assess the rehabilitation potential, the condition of praxis in stage II Parkinson’s disease (Hoehn and Yahr) of mixed form of moderate type of progression, with a mild degree of impairment, without dyskinesia, with moderate depression, subclinical anxiety against the background of the change in visual gnosis in the form of paragnosis, the preservation of the auditory gnosis, i.e. the absence of gross changes «at the entrance» has been studied.
The review describes modern views on consequences of regular alcohol ingestion for human body. The results of alcohol influence on cardiovascular, nervous, gastrointestinal and other systems of the organism were shown. Controversy of protective effects of low alcohol doses was discussed.
An algorithm for early detection of skin melanoma in a district polyclinic is presented. The authors conclude that it is necessary to widely implement excisional biopsy of dysplastic nevi in district polyclinics. All the doctors who consult patients in district polyclinics, as well as the average medical staff should identify dysplastic nevi and send patients to the surgeon to perform a biopsy. An increase in the number of biopsies will lead to an increase in the detection of early melanomas and a decrease in mortality from this disease in Russia.
In 2010–2019, pathomorphological investigations of the skin biopsies were performed in 561 patients with suspected lymphoproliferative skin manifestations. The diagnosis of primary skin lymphomas was verified in 105 patients, of which Mycosis Fungoides was detected in 86 patients. Involvement of additional options for objectifying the results of pathomorphological and immunohistochemical studies of skin biopsies of patients with «complex diagnostic search», including those with suspected primary skin lymphomas, contributes to early diagnostics of the diseases.
Cutaneous lymhangioma circumscriptum (CLC) is an acquired or inherited anomaly of superficial lymphatic vessels that presents as grouped papules with vesicles on their top. These vesicles contain translucent fluid but also might be filled with blood, which makes them look like «frog spawn». The differential diagnosis of CLC includes molluscum contagiosum, angiokeratoma, hemangioma. Besides, considering of cutaneous metastases (carcinoma teleangiectoides) is important in order of similar manifestation. The article presents a clinical case of a 54-year-old woman with CLC occurred after modified radical mastectomy.
The description of the clinical case of a patient with systemic lupus erythematosus, difficulties in diagnostics due to atypical onset and clinical manifestations were presented, recommendations to prevent late diagnosis are given.
The article presents current data on the treatment of patients with psoriasis and psoriatic arthritis using gene-engineered biologic drugs of various classes. The article presents the experience of clinical monitoring of a female patient with an extremely severe variant of the disease course, resistant to standard treatment methods and demonstrating insufficient and short-term effectiveness of the use of biological drugs, who responded to treatment with ixekizumab.
Within the period from 1997 to 2017, diagnostics and treatment were performed on 87 children with cholelithiasis, who were admitted to the District Children’s Clinic of Ufa and City Children’s clinic no. 17. The method of computer tomography with cholekinetic was developed and implemented, and applied on 46 patients with cholelithiasis (CL). It was also revealed that it has a number of advantages as compared to traditional methods of radiodiagnostics, and is one of key criteria in defining indications for surgical treatment of children with CL.
In spite of emergency situation with COVID-19 pandemic, other diseases remain topical, including those of infectious origin. We are currently in the middle of influenza season 2019–2020, which began in September 2019, and will finish in September 2020. In this connection, it is extremely important to continue doing adequate vaccinal prevention in children. One of the main objectives of vaccination strategy is construction of perfect vaccines and improvement of tactics of their application – transition to four-valence preparations, new adjuvants, universal vaccines, creation of collections of strains with pandemic potentials.
The article covers the issues of providing medical assistance to children with diabetes mellitus (DM) and novel coronavirus infection COVID-19. Currently, there are some data that COVID-19 has more mild and asymptomatic forms in children’s population. However, it shouldn’t reduce watchfulness in respect of combination of these diseases, because diabetes mellitus is related to comorbid risk factors. The publication covers the issues of diabetes control and anti-epidemic measures in children with COVID-19. The recommendations on routing the patients depending on severity of coronavirus infection and degree of diabetes decompensation were validated.
