СРК-подобные симптомы при воспалительных заболеваниях кишечника: клиническая необходимость или терминологический компромисс?

Аннотация

Введение. Несмотря на наличие эффективных терапевтических стратегий, направленных на индукцию и поддержание ремиссии воспалительных заболеваний кишечника, у части пациентов имеют место персистирующие гастроинтестинальные симптомы: рецидивирующий абдоминальный болевой синдром, изменение частоты и/или консистенции стула, что нередко трактуется как симптомы, сходные с проявлениями синдрома раздраженного кишечника (СРК). Учитывая гетерогенность возможных причин гастроинтестинальных нарушений и отсутствие валидированных диагностических критериев для данной популяции пациентов, использование термина «СРК-подобные симптомы» при воспалительных заболеваниях кишечника остается предметом дискуссий. Длительно сохраняющиеся симптомы со стороны желудочно-кишечного тракта, а в некоторых случаях и системные проявления могут быть вызваны широким спектром этиологических факторов, включая синдром избыточного бактериального роста, мальабсорбцию желчных кислот и/или олиго-/моносахаридов, экзокринную недостаточность поджелудочной железы, глютен-ассоциированные состояния, эозинофильные гастроинтестинальные заболевания, синдром активации тучных клеток, а также состояния, не связанные с непосредственным вовлечением желудочно-кишечного тракта, в частности, эндометриоз и спаечную болезнь.

Заключение. Учитывая сказанное выше, использование термина «СРК-подобные симптомы» представляется оправданным в случаях, когда у пациента с воспалительными заболеваниями кишечника достигнута гистологическая ремиссия, а сохраняющиеся гастроинтестинальные симптомы обусловлены расстройствами оси «кишечник – мозг». Применение вышеуказанного термина целесообразно при условии исключения иных причин, способных объяснить клинические проявления как со стороны желудочно-кишечного тракта, так и за его пределами.
