Сексуальные дисфункции при тревожных расстройствах

Введение. Являясь одним из широко распространенных психических заболеваний, тревожные расстройства часто сопровождаются сексуальными дисфункциями. Фармакотерапия, применяемая в лечении тревожных расстройств, в свою очередь может приводить к побочным сексуальным нарушениям. О наличии проблем в интимной сфере при обращении за психиатрической помощью сообщает лишь небольшая часть пациентов. Однако несвоевременное выявление сексуальных дисфункций может снижать приверженность терапии и ее эффективность.

Материалы и методы. Статья посвящена обзору, основанному на 21 источнике, в том числе на 9 систематических обзорах и других исследованиях. Поиск данных осуществлялся по названиям, аннотациям и ключевым словам: сексуальная дисфункция, тревожное и стрессовое расстройства (sexual dysfunction, anxiety and stress disorder) в базах данных Elibrary и PubMed.

Результаты. При тревожных расстройствах сексуальные дисфункции наблюдаются как у мужчин, так и у женщин и проявляются нарушениями всех фаз сексуальных реакций. Взаимосвязь сексуальных дисфункций с тревожными расстройствами объясняется тремя моделями. Первая, включающая причинно-следственные связи, предполагает, что корреляция между сексуальной дисфункцией и тревожными расстройствами часто является двунаправленной, причем симптомы сексуального и тревожного расстройства могут проявляться одновременно. Вторая модель предполагает наличие общего основного фактора или скрытой предиспозиции, вызывающих со временем развитие как психических, так и сексуальных расстройств, Третья модель концептуализирует сексуальные дисфункции как нарушения, не связанные с тревожными расстройствами.

Заключение. На основании проведенного исследования авторами рекомендован следующий алгоритм курации пациентов, страдающих тревожными расстройствами и сексуальными нарушениями. На этапе обращения за психиатрической помощью, наряду со стандартными методами диагностики для определения выраженности психической патологии, целесообразно использовать опросники для оценки сексуальной функции. Кроме того, следует учитывать возможное негативное влияние фармакотерапии на сексуальную функцию и оценивать ее на всех этапах лечения для проведения своевременной коррекции. При выявлении сексуальных нарушений у пациентов с тревожными расстройствами с учетом клинической ситуации по возможности назначаются психотропные препараты с минимальным негативным влиянием на сексуальную функцию или без такового. При невозможности применения таких средств после достижения коррекции тревожной симптоматики могут быть снижены дозы используемых психотропных препаратов, рекомендованы перерывы в их приеме, в основном за 1-2 дня до предполагаемого сексуального контакта. При сохранении сексуальных нарушений в схему лечения в качестве антидота добавляется антидепрессант без влияния на сексуальную функцию или препараты для коррекции сексуальных нарушений – ингибиторы фосфодиэстеразы 5-го типа. Психотерапевтические мероприятия, включающие секс-терапию, являются неотъемлемой частью лечения сексуальных дисфункций у пациентов с тревожными расстройствами.
