В исследовании рассматриваются вопросы обоснованности и эффективности применения предлагаемой схемы консервативной терапии клинически выраженных стадий остеоартроза коленного сустава (II-III степени по классификации КеллгренаЛоуренса), включающей сочетанный прием препаратов из разных фармакологических групп: хондропротектора хондроитина сульфата, производного гиалуроновой кислоты и нестероидного противовоспалительного препарата. Целью исследования было оценить эффекты сочетанного применения хондроитина сульфата, деривата гиалуроновой кислоты и ацеклофенака в отношении функционального статуса пораженного сустава в течение всего исследования (30 дней), а также прогрессирования дегенеративно-воспалительного процесса путем оценки объективных данных (УЗИ-картины и лабораторных показателей). Проведено проспективное открытое исследование сочетанной консервативной терапии 69 амбулаторных пациентов за период с 2017 по 2019 гг. с предметной оценкой артикулярной компоненты лечения. В результате изучения клинической эффективности вышеуказанной схемы у пациентов с преобладанием артритического компонента заболевания установлены положительные результаты медикаментозной терапии. К концу наблюдения выраженность болевого синдрома и в большинстве случаев лабораторно-морфологические изменения продемонстрировали статистически значимые позитивные сдвиги, в то время как положительная динамика амплитуды движений не всегда была достоверной. Ни один из пациентов не был исключен из исследования, все больные перенесли проведенный курс терапии удовлетворительно, нежелательных явлений не возникло.
The study discusses issues of the validity and efficacy of the proposed scheme of conservative treatment of clinically significant stages of osteoarthritis of the knee joint (II-III degree classification Kellgren – Lawrence), including a combined administration of drugs from different pharmacological groups: non-steroidal anti-inflammatory drug aceclofenac, a derivative of hyaluronic acid and cartilage chondroitin sulfate. The aim of the study was to assess the effects of the combined use of aceclofenac, hyaluronic acid derivative and chondroitin sulfate on the functional status of the affected joint throughout the study (30 days), as well as the progression of the degenerative inflammatory process by assessing objective data (ultrasound pictures and laboratory parameters). The prospective open-label study of combined conservative therapy in 69 outpatient patients for the period from 2017 to 2019 with a subject assessment of the articular component of the treatment was conducted. As a result of the study of the clinical efficacy of the above treatment regimen in patients with a predominance of the arthritis component of the disease, positive results of drug therapy were established. By the end of the observation the intensity of a pain syndrome and, in most cases, laboratory and morphological changes showed statistically significant positive changes, while positive dynamics of movements, observed mainly in patients with a second stage of gonarthrosis were not always reliable. None of the patients was excluded from the study, all patients underwent a course of therapy satisfactorily, no adverse events occurred.
