Для нее характерно длительное рецидивирующее течение, невысокая эффективность комплексной терапии. Появление сегодня новой патогенетической концепции ось кишечникмикробиотамозг позволило переоценить механизм и старт возникновения висцеральной гиперчувствительности, основного ключевого патогенетического фактора развития СРК у детей. Современные методы обследования позволили по-новому взглянуть на микробиом человека, его функции, роль микробиоты в патогенезе СРК и других болевых функциональных расстройств пищеварения. Применение пробиотических бактерий в медицинских целях имеет многовековую историю, сегодня их эффекты переоцениваются на высокодоказательном уровне, есть возможность подобрать правильный пробиотический штамм для конкретной клинической ситуации. Взаимодействие кишечной микробиоты и ее активных метаболитов с компонентами пищи и витаминами приводит к синергичному клиническому противовоспалительному и иммунному эффекту, направленному на укрепление кишечного барьера. Использование комплексных средств, содержащих пробиотический штамм L. reuteri DSM 17938 с доказанным клиническим эффектом в терапии абдоминальной боли у детей разного возраста, и витамина D, обладающего эпителиотропным и иммунотропным действием, позволяет значительно увеличить эффективность терапии СРК в детской гастроэнтерологической практике и улучшить прогноз течения заболевания в будущем.</p></abstract><trans-abstract xml:lang="en"><p>Irritable bowel syndrome (IBS) is a common pathology in childhood, accompanied by abdominal pain. It is characterized by a long relapsing course, low efficiency of complex therapy. The emergence of a new pathogenetic concept of the «gut-microbiotabrain» axis today made it possible to re-evaluate the mechanism and the onset of the onset of «visceral hypersensitivity», the main key pathogenetic factor in the development of IBS in children. Modern examination methods have made it possible to take a fresh look at the human microbiome, its functions, the role of the microbiota in the pathogenesis of IBS and other painful functional disorders of digestion. The use of probiotic bacteria for medical purposes has a centuries-old history, today their effects are overestimated at a highly indicative level, it is possible to choose the right probiotic strain for a specific clinical situation. The interaction of the gut microbiota and its active metabolites with food components and vitamins leads to a synergistic clinical anti-inflammatory and immune effect aimed at strengthening the intestinal barrier.
