Neisseria meningitidis (менингококк) является причиной заболевания с высоким уровнем летальности во всем мире, что обусловлено эпидемическими или спорадическими случаями. Вирулентность инвазивной N. meningitidis обусловлена несколькими ключевыми факторами, в том числе поверхностными адгезивными белками (пили), защитной капсулой и выделением эндотоксина (липида А из липоолигосахаридов). Кроме того, бактерия в ходе эволюции выработала генетические адаптации, такие как горизонтальный обмен генами, антигенная вариабельность и молекулярная мимикрия, которые позволяют ей успешно колонизировать слизистые оболочки, проникать в кровоток и избегать воздействия иммунной системы. Заражение N. meningitidis обычно происходит воздушно-капельным путем. Менингококковая инфекция может протекать как в виде локализованных (назофарингит, носительство), так и генерализованных форм (менингит, менингококцемия, менингоэнцефалит, смешанные формы). Также выделяют редкие формы – артрит, миокардит, иридоциклит и др. Форма инфекции во многом определяет исход, спектр возможных осложнений и отдаленных последствий, на долю которых приходится до 20-30% случаев. Эпидемиология инфекции, вызываемой N. meningitidis, изменчива и непредсказуема. Ранние клинические симптомы заболевания неспецифические и могут привести к ошибочному диагнозу, а значит, и несвоевременному назначению оптимального лечения.</p></sec><sec><title>Цель работы</title><p>Цель работы. На основании анализа клинических и лабораторных данных установить особенности течения и маркеры неблагоприятного течения менингококкового сепсиса (менингококцемии).</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведен анализ карт стационарных пациентов в возрасте до 18 лет, которые находились на лечении в Учреждении здравоохранения «Городская детская инфекционная клиническая больница» (г. Минск) с менингококцемией (менингококковым сепсисом) с 2009 по 2024 г.</p></sec><sec><title>Результаты</title><p>Результаты. Основными клиническим проявлениями менингококкового сепсиса являются фебрильная лихорадка (98,1-100%) и сыпь (100%). Наиболее часто менингококцемия протекает с сочетанием поражения нервной системы и менингита/менингоэнцефалита (53,2-69,3%). Развитие синдрома Уотерхауса – Фридериксена при менингококковом сепсисе увеличивает риск смерти почти в 9 раз.</p></sec><sec><title>Заключение</title><p>Заключение. Установленными маркерами неблагоприятного течения менингококкового сепсиса являются септический шок и синдром Уотерхауса – Фридериксена.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Neisseria meningitidis (meningococcus) is a highly fatal disease worldwide, occurring in epidemic and sporadic cases. The virulence of invasive N. meningitidis is determined by several key factors, including surface adhesins (pili), a protective capsule, and endotoxin release (lipid A from lipooligosaccharides). Furthermore, the bacterium has evolved genetic adaptations, including horizontal gene transfer, antigenic variation, and molecular mimicry, which allow it to successfully colonize mucosal surfaces, invade the bloodstream, and evade the immune system. The acquisition of N. meningitidis typically occurs through exposure to respiratory droplets. Meningococcal infection can present in both localized (nasopharyngitis, carriage) and generalized forms (meningitis, meningococcal bloodstream infection, meningoencephalitis, and mixed forms). Atypical presentations also include: arthritis, myocarditis, iridocyclitis, and others. The clinical form of infection is a key determinant of the outcome, the spectrum of potential complications and long-term sequelae, accounting for approximately 20-30% of cases. The epidemiology of N. meningitidis is variable and unpredictable. Early clinical symptoms are nonspecific and can lead to misdiagnosis and, therefore, delayed optimal treatment.</p></sec><sec><title>Objective</title><p>Objective. To analyze clinical and laboratory data to identify the clinical features and markers of adverse events in meningococcal sepsis (meningococcemia).</p></sec><sec><title>Materials and methods</title><p>Materials and methods. We analyzed the records of inpatients under 18 years of age treated for meningococcemia (meningococcal sepsis) at the Minsk City Children's Infectious Diseases Clinical Hospital from 2009 to 2024.</p></sec><sec><title>Results</title><p>Results. The main clinical manifestations of meningococcal sepsis are febrile fever (98.1-100%) and rash (100%). Meningococcemia most often occurs in combination with nervous system damage and the development of meningitis/meningoencephalitis (53.2-69.3%). The development of Waterhouse-Friderichsen syndrome in meningococcal sepsis increases the risk of death by almost 9 times. Conclusion.
