Одной из значимых проблем ранней реабилитации пациентов с острым нарушением мозгового кровообращения является развитие гемодинамических нарушений, вызванных изменением потоков в системе базальных анастомозов. В результате активно проводимая физическая реабилитация может не только не достигать ожидаемого эффекта, но и приводить к обратному результату. Решением данной проблемы мог бы стать метод динамического наблюдения за состоянием церебральной гемоперфузии, который можно было бы применять в том числе и в условиях проведения реабилитационных мероприятий для наблюдения за изменениями в состоянии пациента во время выполнения подобранной ему физической нагрузки. В нашем исследовании изучалась возможность использования метода клинической электроэнцефалографии для решения данной задачи, поскольку методика широко распространена в современном здравоохранении, не требует значительных затрат при проведении исследования и не оказывает неблагоприятного действия на пациента.</p></sec><sec><title>Цель работы</title><p>Цель работы. Цель представленной работы состоит в описании возможности исследования ритмических медленноволновых феноменов, связанных с развитием нарушений локальной гемоперфузии, возникающих при повышении физической активности у пациентов в раннем восстановительном периоде ишемического инсульта.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Было обследовано 24 человека, перенесших ишемический инсульт (атеротромботический вариант) в течение года и имевших индекс Рэнкина – 3. Средний возраст обследованных составил 57,3 года, Мо – 55, Ме – 58, первый квартиль – 55, третий – 61,3, возрастной диапазон – 38 лет, минимальный возраст – 35 лет, максимальный – 73 года.</p></sec><sec><title>Результаты</title><p>Результаты. Установлено, что в первые 6 месяцев физическое воздействие на пораженные конечности вызывают значимые изменения церебральной гемодинамики, которые могут приводить к развитию гемодинамического обкрадывания смежных областей коры головного мозга, в том числе и в непораженном полушарии. Эти данные необходимо учитывать при разработке программ физической реабилитации пациентов с острым нарушением мозгового кровообращения, а сам метод электроэнцефалографии может с успехом использоваться для непосредственного наблюдения за состоянием церебральной гемоперфузии, в том числе в условиях проведения реабилитационных мероприятий.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. One of the significant problems of early rehabilitation of acute cerebrovascular accident is the development of hemodynamic disorders caused by changes in flows in the system of basal anastomoses. As a result, actively conducted physical rehabilitation may not only fail to achieve the expected effect, but also lead to the opposite result. A solution to this problem could be a method of dynamic monitoring of the state of cerebral hemoperfusion, which could be used, including in the context of rehabilitation measures, to monitor changes in the patient's condition during the physical load imposed on him. In our study, we examined the possibility of using the clinical electroencephalography method to solve this problem, since the technique is widely used in modern healthcare, does not require significant costs for the study and does not have an adverse effect on the patient.</p></sec><sec><title>Objective</title><p>Objective. The purpose of the presented work is to describe the possibility of studying rhythmic slow-wave phenomena associated with the development of local hemoperfusion disorders that occur with increased physical activity in patients in the early recovery period of ischemic stroke.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. We examined 24 people who had suffered an ischemic stroke (atherothrombotic variant) during the year, who had a Rankin index of 3. The average age of the examined was 57.3 years, Mo – 55, Me – 58, First quartile – 55, third – 61.3. Age range – 38 years. Minimum age – 35 years, maximum – 73 years.</p></sec><sec><title>Results</title><p>Results. Our study found that during the first 6 months, physical impact on the affected limbs causes significant changes in cerebral hemodynamics, which can lead to the development of hemodynamic steal in adjacent areas of the cerebral cortex, including in the unaffected hemisphere.
