С каждым годом растет заболеваемость аутоиммунными нарушениями. Важно отметить, что зачастую первые признаки патологии возникают на слизистых ротовой полости. Среди дерматологических нозологий к таковым относится вульгарная пузырчатка. Этот тип заболевания в 50% случаев начинается именно с высыпаний в полости рта, при этом эрозивно-язвенные дефекты слизистой оболочки ротовой полости наблюдаются в 70-90% случаях данной патологии. Этиология вульгарной пузырчатки неясна. С точки зрения патофизиологии пузырчатка проявляется развитием аутоантител против межклеточных веществ. Клинические проявления характеризуются образованием внутриэпителиальных пузырей на коже и слизистых оболочках.</p></sec><sec><title>Результаты</title><p>Результаты. Первой линией терапии являются топические и системные глюкокортикостероиды, в случаях тяжелых форм и торпидного течения в схему лечения включают иммуносупрессивные препараты. Локализация высыпаний в ротовой полости существенно сужает ряд доступных для терапии препаратов. Кроме того, из-за особенностей анатомической области высок риск быстрого присоединения вторичной инфекции.</p></sec><sec><title>Заключение</title><p>Заключение. Одним из препаратов, доступных к применению при эрозивно-язвенных дефектах полости рта, является комбинация 0,1%-го ундециленового амидопропил-бетаина и 0,1%-го полиаминопропила бигуанида (полигексанид). Антисептик полигексанид уже много десятилетий применяется в медицине. Он бесцветен, не имеет запаха, растворим в воде и спирте, демонстрирует бактерицидную и фунгицидную активность. Собственный опыт показывает его высокую эффективность и хорошую переносимость у пациентов с проявлениями вульгарной пузырчатки в полости рта. Значимый клинический эффект достигается после двух недель применения. Использование препарата хорошо сказывается на качестве жизни пациентов, учитывая, что болезненные эрозивные высыпания значительно влияют на возможности потребления пищи и доставляют сильный дискомфорт.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. The incidence of autoimmune diseases is increasing every year. It should be mentioned that early signs of the pathology often emerge on the oral mucosa. Among dermatological conditions, pemphigus vulgaris is classified as such. This type of the disease starts with oral rashes in 50% of cases. However, erosive and ulcerative lesions of the oral mucosa occur in 70-90% of cases. The etiology of pemphigus vulgaris is unclear. From a pathophysiological perspective, pemphigus manifests with the development of autoantibodies against intercellular substance. Clinical features present as intraepithelial blister formation on the skin and mucous membranes.</p></sec><sec><title>Results</title><p>Results. Topical and systemic glucocorticosteroids are the first-line therapy. In cases of severe forms and torpid course, the treatment regimen includes immunosuppressive drugs. The localization of eruptions in the oral cavity significantly limits the range of available therapeutic options. Furthermore, due to the anatomical features of the region, there is a high risk of rapid secondary infection.</p></sec><sec><title>Conclusion</title><p>Conclusion. One of the medications accessible for the treatment of erosive and ulcerative lesions of the oral cavity is a combination of undecylenamidopropyl betaine at a concentration of 0.1% and Polyhexamethylene biguanid (polyhexanide) at a concentration of 0.1%. Polyhexanide is an antiseptic. It has been used in clinical practice for decades. It is colorless, odorless, soluble in water and alcohol and possesses bactericidal and fungicidal properties. Our experience indicates its high efficacy and good tolerability in patients with oral manifestations of pemphigus vulgaris. Clinically significant improvement is observed after two weeks of therapy.
