В лечении cахарного диабета 2-го типа были предложены инновации. Метформин по-прежнему рекомендуется как препарат первой линии, но расширяются показания к использованию агонистов рецепторов глюкагоноподобного пептида 1 и ингибиторов натрий-глюкозного котранспортера-2. В обзорных статьях не всегда отмечается, что гипогликемический эффект агонистов рецепторов глюкагоноподобного пептида-1 связан со стимуляцией инкреторной функции бета-клеток, которая со временем может истощаться.</p></sec><sec><title>Цель работы</title><p>Цель работы. Объективное сравнение эффективности и возможных побочных эффектов лекарственных препаратов, используемых при лечении cахарного диабета 2-го типа с избыточной массой тела.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Обзор отечественной и зарубежной литературы с использованием баз данных elibrary.ru, PubMed и Google Scholar, а также библиотечных каталогов.</p></sec><sec><title>Результаты</title><p>Результаты. Представлен краткий обзор недавних публикаций о сахароснижающих препаратах, которые могут применяться при сахарном диабете 2-го типа у пациентов с избыточной массой тела. Обсуждаются влияние лекарств на массу тела и функцию бета-клеток, а также сравнительная стоимость лечения. Особое внимание уделено следующим группам лекарственных средств. Агонисты рецепторов глюкагоноподобного пептида-1 стимулируют секрецию инсулина, снижают аппетит и моторику желудочно-кишечного тракта, способствуют уменьшению массы тела. Отмечена сравнительно высокая стоимость агонистов рецепторов глюкагоноподобного пептида-1 и подкожное введение. Пероральный семаглутид является пока единственным агонистом рецепторов глюкагоноподобного пептида-1 для приема внутрь. Препараты ингибиторов натрий-глюкозного котранспортера-2 уменьшают почечную реабсорбцию глюкозы, выводят глюкозу с мочой, снижают артериальное давление и способствуют уменьшению веса. Гликемический контроль поддерживается на протяжении длительного времени. Снижения массы тела можно ожидать также от ингибиторов кишечной альфа-глюкозидазы (акарбоза), тормозящих переваривание сложных углеводов. Гипогликемический эффект препаратов двух последних групп (а также метформина) не связан со стимуляцией инкреторной функции бета-клеток. Обсуждается комбинированная терапия с использованием сахароснижающих и анорексигенных препаратов.</p></sec><sec><title>Заключение</title><p>Заключение. Осложнения сахарного диабета обусловлены не только гипергликемией, но и дислипидемией, артериальной гипертензией, гиподинамией, курением и другими факторами, что требует индивидуального подхода к медикаментозной терапии и модификации образа жизни.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Innovations have been recently proposed in the treatment of type 2 diabetes. Metformin is recommended as a first-line drug now as before; but indications for the use of glucagon-like peptide 1 receptor agonists and sodium-glucose cotransporter 2 inhibitors are expanding. Review articles do not always mention that the hypoglycemic effect of glucagon-like peptide 1 receptor agonists is associated with the stimulation of the endocrine function of beta cells, which may be depleted over time.</p></sec><sec><title>Objective</title><p>Objective. Unbiased comparison of the effectiveness and possible side effects of drugs used in the treatment of type 2 diabetes mellitus with overweight.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. Review of Russian and international literature using the databases elibrary.ru, PubMed and Google Scholar, as well as library catalogs.</p></sec><sec><title>Results</title><p>Results. A brief overview of recent publications on sugar-lowering drugs that can be used in type 2 diabetes with overweight patients is presented. The effect of drugs on body weight and beta cell function, as well as relative cost of the treatment is discussed. Special attention is paid to the following groups of medicines. Glucagon-like peptide 1 receptor agonists stimulate the secretion of insulin, lowering appetite and the gastrointestinal motility, thus promoting weight loss. The relatively high cost of glucagon-like peptide 1 receptor agonists and subcutaneous administration are pointed out. Oral semaglutide is the only glucagon-like peptide 1 receptor agonist preparation used for the oral intake. Sodium-glucose cotransporter 2 inhibitors reduce renal glucose reabsorption, excrete glucose in the urine, lower blood pressure, and promote weight loss. Glycemic control is thus maintained in the long term. Weight loss can also be expected from intestinal alpha-glucosidase inhibitors (acarbose), which inhibit the digestion and absorption of complex carbohydrates in the intestine. The hypoglycemic effect of the drugs of last two groups (as well as metformin) is not associated with the stimulation beta cells, which can be depleted by prolonged stimulation. Combination therapy using sugar-lowering and anorexigenic drugs is discussed.</p></sec><sec><title>Conclusion</title><p>Conclusion.
