Лечение хронических ран – это существенная клиническая, социальная и экономическая проблема, и, учитывая тенденции старения населения, сохраняющуюся угрозу сахарного диабета и ожирения во всем мире, а также сохраняющуюся проблему инфекций, ожидается, что проблема продолжит быть актуальной, требующей коррекции парадигмы подходов к диагностике и лечению. До сих пор в медицинском сообществе существуют проблемы в принятии единой терминологии, диагностике причин хронизации ран и выборе оптимальной тактики их лечения. Оптимальный подход к лечению хронических ран основывается прежде всего на диагностике причин замедления заживления и применении средств, которые эффективно и безопасно его ускоряют. Известно, что коллаген и его соединения, находясь во внеклеточном матриксе, влияют на все ключевые этапы процесса заживления ран. В то же время нарушения в регуляции обмена коллагенов в ране приводят к патологическим состояниям, таким как фиброз и хронизация. В связи с этим баланс активности протеолитических ферментов (коллагеназ) и их ингибирования также необходим для нормального заживления ран. Повязки на основе коллагена представляют собой покрытия, отвечающие всем основным требованиям современного перевязочного материала: атравматичность, обеспечение баланса влажности, защита от инфекции, биодеградация, простота использования, экономическая эффективность и др. Применение коллагеновой повязки может быть эффективно на всех стадиях раневого процесса: гемостаз, воспаление, пролиферация и ремоделирование, – поскольку на всех стадия коллаген влияет на процессы заживления. При добавлении дополнительных лекарственных средств коллагеновая повязка приобретает гемостатические свойства, может быть использована для очистки раны ферментами и способствует гармоничной пролиферации в ране в условиях избыточной продукции протеолитических ферментов.</p></sec><sec><title>Результаты</title><p>Результаты. В публикации представлены результаты клинического наблюдения применения биологической повязки с природными коллагеназами и коллагеновой ранозаживляющей повязки, в состав которых входит негидролизованный коллаген. Применение исследуемых препаратов оказало значимое влияние на скорость заживления хронических ран у пациентов, вошедших в клиническое наблюдение.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. The treatment of chronic wounds is a significant clinical, social, and economic problem. Given the aging population, the continuing threat of diabetes and obesity worldwide, and the ongoing problem of infections, this issue is expected to remain relevant, requiring a paradigm shift in approaches to diagnosis and treatment. To date, the medical community has struggled to agree on uniform terminology, diagnose the causes of chronic wounds, and select the optimal treatment tactics. The optimal approach to treating chronic wounds is based primarily on diagnosing the causes of slow healing and using agents that effectively and safely accelerate it. It is known that collagen and its compounds, located in the extracellular matrix, influence all key stages of the wound healing process. At the same time, disturbances in the regulation of collagen metabolism in the wound lead to pathological conditions such as fibrosis and chronicity. In this regard, the balance between the activity of proteolytic enzymes (collagenases) and their inhibition is also necessary for normal wound healing. Collagen-based dressings are coverings that meet all the basic requirements of modern dressing materials: atraumaticity, moisture balance, protection against infection, biodegradability, ease of use, cost-effectiveness, etc. Collagen dressings can be used effectively at all stages of the wound healing process: hemostasis, inflammation, proliferation, and remodeling, as collagen influences healing processes at all stages. When additional drugs are added, the collagen dressing acquires hemostatic properties, can be used to clean the wound with enzymes, and promotes harmonious proliferation in the wound in conditions of excessive production of proteolytic enzymes.</p></sec><sec><title>Results</title><p>Results. The publication presents the results of clinical observation of the use of preparations: the biological dressing with natural collagenases and the collagen wound healing dressing, which contain non-hydrolyzed collagen.
