Актуальной проблемой в современной эпилептологии являются коморбидные расстройства при эпилепсии. В клинической практике специалисты встречаются с различными психопатологическими расстройствами у пациентов с эпилепсией, среди которых особенно часто наблюдаются тревожные и депрессивные состояния. Не менее значимыми являются конверсионные (диссоциативные) расстройства. Данные нарушения представляются труднокурабельными ввиду наличия комплекса факторов, приводящих к их развитию.</p></sec><sec><title>Цель работы</title><p>Цель работы. Изучить распространенность аффективных и конверсионных расстройств и методы их лечения у пациентов с эпилепсией.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведен анализ научных исследований, посвященных взаимосвязи эпилепсии, аффективных и конверсионных расстройств и методов их лечения.</p></sec><sec><title>Результаты</title><p>Результаты. Несмотря на широкие диагностические и терапевтические возможности, у 25-30% пациентов не достигается контроль приступов с помощью фармакотерапии. У 60-80% пациентов приступы сохраняются на фоне противоэпилептической терапии. Известно, что 60% пациентов с персистирующими приступами имеют аффективные расстройства. Депрессивная симптоматика занимает лидирующее место и встречается у 7,6-41,4% пациентов, а при фармакорезистентности – до 86%. Также у пациентов с фармакорезистентной эпилепсией выявляются конверсионные (диссоциативные) расстройства, проявляющиеся в виде психогенных неэпилептических приступов. Однако у пациентов с психогенными неэпилептическими приступами имеет место псевдофармакорезистность – им назначается противоэпилептическая терапия, не оказывающая должного эффекта в отношении психогенных неэпилептических приступов, что ухудшает клинический прогноз. До 40% пациентов, поступающих в клиники с диагнозом «фармакорезистентная эпилепсия», имеют сочетание эпилептических приступов и психогенных неэпилептических приступов.</p></sec><sec><title>Заключение</title><p>Заключение. Подбор противоэпилептической терапии должен проводиться с учетом не только типов приступов, но и коморбидных психических расстройств. Наиболее предпочтительными при аффективных и конверсионных расстройствах являются антиконвульсанты с нормотимическим эффектом, к примеру лакосамид. Данный препарат имеет широкий спектр действия на эпилептические приступы и обладает выраженным нормотимическим эффектом.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Comorbid disorders in epilepsy are an urgent problem in modern epileptology. In clinical practice, specialists encounter various psychopathological disorders in patients with epilepsy, among which anxiety and depressive states are especially common. No less significant are conversion (dissociative) disorders. These violations seem difficult to resolve due to the presence of a complex of factors leading to their development.</p></sec><sec><title>Objective</title><p>Objective. The aim is to study the prevalence of affective and conversion disorders and their treatment methods in patients with epilepsy.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The analysis of scientific studies on the relationship between epilepsy, affective and conversion disorders and their treatment methods has been carried out.</p></sec><sec><title>Results</title><p>Results. Despite the wide diagnostic and therapeutic possibilities, 25-30% of patients do not achieve seizure control with the help of pharmaco therapy. In 60-80% of patients, seizures persiston the background of antiepileptic therapy (PET). It is known that 60% of patients with persistent seizures have affective disorders. Depressive symptoms occupy a leading place and occur in 7.6-41.4% of patients, and with pharmacoresistance – up to 86%. Also, patients with pharmacoresistant epilepsy (PE) have conversion (dissociative) disorders, manifested in the form of psychogenic non-epileptic seizures (PNEP). However, patients with PNEP have "pseudo pharmacoresistance" – patients are prescribed PET, which does not have the desired effecton PNEP, worsening the clinical prognosis. Up to 40% of patients admitted to clinics with a diagnosis of "pharmacoresistant epilepsy" have a combination of epileptic seizures and NEP.</p></sec><sec><title>Conclusion</title><p>Conclusion. When choosing PET, you should considernotonly the types of seizures, but also the concomitant mental disorders. Anticonvulsant drugs with anormothymic effect, for example, lacosamide, are the most preferred for affective and conversion disorders.
