При нейроинфекциях вследствие бактериемии или вирусемии возникает генерализованный васкулит, в том числе и церебральных сосудов, что обусловливает развитие дисфункции эндотелия. Это способствует возникновению тромбоза сосудов, нарушению сосудистой регуляции, стимулирует нейровоспаление за счет увеличения секреции провоспалительных цитокинов из иммунных и нейрональных клеток, таких как микроглия и астроциты. Предполагается, что активация микроглии играет важную роль в гибели нейрональных клеток за счет высвобождения провоспалительных медиаторов. Ведущую роль в развитии ответа острой фазы играют интерлейкины, интерфероны, фактор некроза опухолей альфа и факторы, активизирующие и тормозящие деятельность макрофагов.</p></sec><sec><title>Цель работы</title><p>Цель работы. Проведено сравнение лабораторных показателей уровня лейкоцитов в периферической крови и биохимических маркеров воспаления (С-реактивный белок, ферритин, интерлейкин-6 в сыворотке крови) и уровня интерлейкина-6 в ликворе у пациентов с менингитом различной этиологии и больных без воспалительных изменений в центральной нервной системе. В исследовании участвовал 151 пациент, госпитализированный в ГБУЗ НСО «ГИКБ № 1» в 2023-2024 гг. с подозрением на серозный или гнойный менингит. У всех больных проводилась оценка лабораторных показателей крови и спинномозговой жидкости, по результатам которых пациенты были разделены на две группы: больные нейроинфекциями различной этиологии с плеоцитозом в ликворе более 15 и пациенты без воспалительных изменений в центральной нервной системе.</p></sec><sec><title>Результаты</title><p>Результаты. В ходе исследования у пациентов с нейроинфекциями выявлено увеличение в различной степени показателя интерлейкина-6 в цереброспинальной жидкости, установлена прямая корреляционная связь между уровнем плеоцитоза и значениями интерлейкина-6 в ликворе. Также выявлено статистически значимое увеличение концентрации интерлейкина-6 в цереброспинальной жидкости в острой стадии гнойных и серозных менингитов, что свидетельствует о непосредственной выработке интерлейкина-6 клетками нейроглии, выполняющими функцию макрофагов в центральной нервной системе.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. In neuroinfections due to bacteremia or viralemia, generalized vasculitis occurs, including cerebral vessels, which causes the development of endothelial dysfunction. This contributes to vascular thrombosis, impaired vascular regulation, and stimulates neuroinflammation by increasing the secretion of proinflammatory cytokines from immune and neuronal cells such as microglia and astrocytes. Microglia activation is thought to play an important role in neuronal cell death through the release of proinflammatory mediators. Interleukins, interferons, tumor necrosis factor alpha, and factors that activate and inhibit macrophage activity play a leading role in the development of the acute phase response.</p></sec><sec><title>Objective</title><p>Objective. Comparison of laboratory parameters of the level of leukocytes in the peripheral blood and biochemical markers of inflammation (CRP, ferritin, IL-6 in the blood serum) and the level of IL-6 in the cerebrospinal fluid in patients with meningitis of various etiologies and patients without inflammatory changes in the central nervous system. The study involved 151 patients hospitalized at the State Budgetary Healthcare Institution of the Novosibirsk region "GIKB No. 1" in 2023-2024 with suspected serous or purulent meningitis. In all patients, laboratory parameters of blood and CSF were assessed, according to the results of which the patients were divided into two groups: patients with neuroinfections of various etiologies with pleocytosis in the cerebrospinal fluid of more than 15 and patients without inflammatory changes in the central nervous system.</p></sec><sec><title>Results</title><p>Results. The study revealed an increase in varying degrees of IL-6 in the cerebrospinal fluid in patients with neuroinfections, and a direct correlation was established between the level of pleocytosis and IL-6 values in the cerebrospinal fluid.
