Неалкогольная жировая болезнь печени на сегодняшний день является наиболее распространенным хроническим заболеванием печени во всем мире с глобальной распространенностью 32,4%. В настоящее время ведется активный поиск новых терапевтических возможностей ведения пациентов с неалкогольной жировой болезнью печени. Вместе с тем идет процесс переосмысления действия известных препаратов, в частности урсодезоксихолевой кислоты, с позиций новых точек терапевтического применения и метаболических эффектов данного препарата. Урсодезоксихолевая кислота является естественной гидрофильной нецитотоксичной желчной кислотой, которая присутствует в норме в составе желчи, занимая 3-5% пула желчных кислот. Урсодезоксихолевая кислота представляет собой препарат с множественными терапевтическими эффектами, который активно применяется в лечении неалкогольной жировой болезни печени. В представленной статье делается акцент на влияние урсодезоксихолевой кислоты на показатели липидного, углеводного обмена, инсулинорезистентность у пациентов с неалкогольной жировой болезнью печени. Отдельное внимание занимает влияние урсодезоксихолевой кислоты на состояние кишечной микробиоты и поддержание целостности кишечного барьера как фактора прогрессии метаболических нарушений при неалкогольной жировой болезни печени.</p></sec><sec><title>Результаты</title><p>Результаты. Данные современной литературы убедительно показывают, что наиболее исследованным, доступным в клинической практике и целесообразным подходом к лечению неалкогольной жировой болезни печени является применение урсодезоксихолевой кислоты. Отмечены доказательные преимущества оригинального препарата урсодезоксихолевой кислоты в коррекции метаболических нарушений у пациентов с неалкогольной жировой болезнью печени.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Nonalcoholic fatty liver disease is by far the most common chronic liver disease worldwide with a global prevalence of 32.4%. Currently, there is an active search for new therapeutic options for the management of patients with nonalcoholic fatty liver disease. At the same time, there is a process of rethinking the effect of known drugs, in particular ursodeoxycholic acid, from the perspective of new points of therapeutic application, including the metabolic effects of this drug. Ursodeoxycholic acid is a naturally occurring hydrophilic non-cytotoxic bile acid that is present normally in bile, occupying 3-5% of the bile acid pool. Ursodeoxycholic acid is currently a drug with multiple therapeutic effects, which is actively used in the treatment of one non-alcoholic fatty liver disease. The article focuses on the effect of ursodeoxycholic acid on lipid, carbohydrate metabolism, and insulin resistance in patients with nonalcoholic fatty liver disease. Special attention is paid to the effect of ursodeoxycholic acid on the state of the intestinal microbiota and the maintenance of the integrity of the intestinal barrier as a factor in the progression of metabolic disorders in non-alcoholic fatty liver disease.</p></sec><sec><title>Results</title><p>Results. The data of modern literature convincingly show that the most researched, available in clinical practice and reasonable approach to the treatment of nonalcoholic fatty liver disease is the use of ursodeoxycholic acid.
